Hypertension and diabetes rarely fail suddenly. They drift, over weeks, while nobody is measuring. This service exists to make that drift visible: medication given correctly, readings taken at every visit, and a documented trend a doctor can actually treat instead of a single reading taken on a bad morning.

- Can the nurse give injections at home?
- Yes. Our nurses are licensed to administer prescribed subcutaneous and intramuscular injections, including insulin and anticoagulants, and to record each dose.
- Will our doctor be able to see the readings?
- Yes. The record can be shared with the treating doctor, and many families bring it to clinic appointments so treatment decisions are based on weeks of home readings rather than one measurement.
- Do you supply the blood pressure machine and glucometer?
- Our nurses attend with their own clinical equipment. Where a household needs its own device between visits, we advise on what to buy and show the family how to use it correctly.
- Can monitoring be added to another care plan?
- It is included as standard in every plan we run. As a standalone service, it suits an otherwise independent relative whose chronic condition simply needs watching.
