Visits run to an agreed monthly rhythm. The same nurse wherever possible, so trust and familiarity build over time.

Every visit is documented
Arrival and departure are timestamped. Vital signs, medication, meals, mood, observations and photos are recorded in the secure portal before the nurse leaves.

You see it, from anywhere
The record reaches your phone the same day. Supervisors review it too, and anything clinically concerning is escalated to you and the doctor immediately.
